The following forms are available from your school or the School Curriculum and Standards Authority at relevant times:
- Application for Equitable Access Adjustments (Special Examination Arrangements) Form Due 27 March 2025.
- Sickness/Misadventure Application Form Due 30 October 2025 (practical examinations) and 25 November 2025 (written examinations).

- EAL/D - Eligibility guide and allocation of extra time for the OLNA (for use from 2023)

A Year 12 student can only enrol in the Year 12 ATAR English as an Additional Language or Dialect (EAL/D) course and receive an extra 10 minutes for the OLNA if they have eligibility approval from their school principal. Students must submit an eligibility declaration in the student portal and their school will then process their declaration in the Student Records management System (SRMS).

The Year 11 Equivalent studies application for a Western Australian Certificate of Education (WACE) form is now available electronically through the Student Records Management System (SRMS) database. A guide to this process can be found on the Year 11 equivalent studies page.
